This Service Request Form is exclusively and specifically for currently-active and/or expired account holders of ParentTaughtDrivingCourse.com, who need one (1) of the following services; 

 
1). Request temporary access to an expired account to VERIFY completion of the state-mandated driving requirements (14 & 30 Hour Driving Logs) and to Request the final DE964 (License ONLY) Certificate of Completion; or
 
2). Request a digital COPY of the DE964 Certificate of Completion; or
 
3). Request a “Transfer” to a Commercial Driving School; or
 
4). Request a Change, Edit or Update to a currently active account or to a DE964 Certificate of Completion
 

Please Note: There are NO Refunds for inadvertent or accidental service requests.

Once we process your request, your original account will be recovered, activated and temporarily activated for three (3) days from your $25 receipt order date, to allow you time to request, download and save your DE964 certificate.

Your request MUST be submitted to Support@ParentTaughtDrivingCourse.com and MUST include the Student’s Full NameDOB, and the Account Username

 
We cannot process your request until we receive the Student’s Full NameDOB, and the Account Username 
 
After we receive your service request email, please allow a minimum of 24 hours for us to begin reviewing your request order.

IMPORTANT NOTICE:

This course is a Parent Taught Driving Course, and it is the “Designated Parent-Instructor’s” responsibility to monitor their student’s course progress and to make sure that their student complies with all TDLR & DPS Driver Education and Safety Administrative Rules and Requirements.

Additionally, if your student did NOT complete the state-mandated course requirements (Modules 1-12 and the 14 & 30 Hour Driving Logs), they will need to be re-enrolled to complete any pending course requirements.

REQUEST your final DE964 (License) Certificate of Completion: 

Follow the Steps Below to Request Your Final DE964 (License) Certificate of Completion:

  1. Send us an email to Support@ParentTaughtDrivingCourse.com
  2. Subject Line: “DE964 Certificate Request”
  3. Include the Registered Student’s Full Legal Name & DOB
  4. Include your Service Request Order Number
  5. Include the original account Username
  6. We cannot process your request without the student’s Legal Name & Original Username
  7. Please note that you will need to VERIFY completion of all course requirements by completing the checkboxes protocol, before being given access to the final DE964 (License) Certificate of Completion.

REQUEST a TRANSFER to a Commercial School:

Follow the Steps Below to Request a Transfer to a Commercial Driving School:

  1. Send us an email to Support@ParentTaughtDrivingCourse.com
  2. Subject Line: “Transfer to a Commercial Driving School”
  3. Include your Service Request Order Number
  4. Include the Registered Student’s Full Legal Name & DOB
  5. Include the Original Account Username
  6. Include the Student’s 8 digit Learner License (permit) Number
  7. Include the Name of the Commercial Driving School to which You’re Transferring
  8. Include the Address of the Commercial Driving School to which You’re Transferring
  9. We cannot process your request without the student’s Legal Name & Original Username
 
REQUEST a Change, Edit or Update to your Account Information
1. Send us an email to Support@ParentTaughtDrivingCourse.com
2. Subject Line: “Change, Edit or Update to Account Information”
3. Include your Service Request Order Number
4. Include the Account Username, Student’s Full Name, & Date of Birth
5. Include details on what exactly needs to be “Updated”

Please allow at least 24 hours for a reply, once we receive your email with the required information; requests received Friday’s after 6 p.m. CST or on Weekends & Holidays, will be reviewed the next business day.
