There are NO
Refunds for inadvertent or accidental “$25 Service Fee,” orders.
This Service Request Order is specifically
for previous account holders of PT #121, who have already completed (Modules 1-12 and the 14 & 30 Hour Driving Logs), but whose account is expired and need either a copy of their DE964 certificate of completion or
a “Transfer” to a Commercial Driving School.
This order allows a onetime
request of ONLY one (1)
the following three options:
- Request for an Original DE964 Certificate of Completion for PT Course #121; or
- Request for a Copy of an already issued DE964 Certificate of Completion for PT Course #121; or
- Request for a “Transfer” DE964 Certificate of Completion to a Commercial Driving School
Make sure that your student was enrolled in our course before
paying the $25 service fee.
Once you complete the Service Fee registration, you will receive an email-receipt with instructions.
