This Service Request order is specifically for previous account holders of PT #121, who have already completed all state-mandated, parent taught driver education course requirements (Modules 1-12 and the 14 & 30 Hour Driving Logs), but whose account is expired and need either a copy of their DE964 certificate of completion or a “Transfer” to a Commercial Driving School.

This service request order allows a onetime request of ONLY one (1) the following three options;

Orders received on Fridays after 6pm CST or on Weekends and Holidays will be processed the next business day.

IMPORTANT NOTICE:

This course is a Parent Taught Driving Course, and it is the “Parent-Instructor’s” responsibility to monitor their student’s course progress and to make sure that their student complies with all TDLR & DPS Driver Education and Safety Administrative Rules and requirements.

 

REQUEST a DE964 (Learner License or License) Certificate – Copy:

Follow the Steps Below to Request a Copy of Your DE964 Certificate of Completion

  1. Send us an email at Support@ParentTaughtDrivingCourse.com
  2. Subject Line: “DE964 – Copy Request”
  3. Include the Registered Student’s Full Legal Name & DOB
  4. Include the New Order Number
  5. MUST Include the Original Account Username
  6. We cannot process your request without the student’s Legal Name & Original Username

Please allow at least 24 hours for a reply, once we receive your email with the required information; requests received Friday’s after 6 p.m. CST or on Weekends & Holidays, will be reviewed the next business day. Important if your student did NOT complete the course requirements (Modules 1-12 and the 14 & 30 Hour Driving Logs), they will need to re-enroll and complete the

Follow the Steps Below to Request a Transfer to a Commercial Driving School:

  2. Subject Line: “Transfer to a Commercial Driving School”
  3. Include the Registered Student’s Full Legal Name & DOB
  4. Include your New Order Number
  5. MUST Include the Original Account Username
  6. Include the Name of the Commercial Driving School to which You’re Transferring
  7. Include the Address of the Commercial Driving School to which You’re Transferring
  8. We cannot process your request without the student’s Legal Name & Original Username

REQUEST a DE964 (License ONLY) Certificate:

Follow the Steps Below if you completed the course requirements, but your account expired before you were able to request your final DE964 Certificate:

  2. Subject Line: “DE964 – Original Request”
  3. Include the Registered Student’s Full Legal Name & DOB
  4. Include your New Order Number
  5. MUST Include the Original Account Username
  6. We cannot process your request without the student’s Legal Name & Original Username
